The video option has disappeared from my iPhone camera. I have Time-lapse, Photo, Square and Pano. It came back momentarily the other day but has since disappeared again.
Any ideas?

If you have it powered up 24/7 my first suggestion is the old switch off/on. Give it a few minutes off.

Rebooting the iPhone is the first thing to do. If just a straight reboot doesn't work then try a soft reset - press and hold the power and home buttons until the iPhone turns off and you see the Apple logo. That's assuming it's still the 5S. Later iPhones are different.

I have a 5S, and I've never used slow mode or for that matter timelapse. Is there any way to adjust the speed or frequency of these? I don't see anything to control them, but perhaps I'm not looking in the right place

The slo-mo is, I believe, shot at 60fps. Only a portion of the video actually shows as slo-mo when you first view it. If you open the file in QT, you can adjust the slo-mo portion as you wish. The time-lapse varies as to the length of the video as below:
